Referrals

Any student can request a check-in with their school counselor. We offer several referral request forms that can be accessed below.

Student Self Referral: Students can complete a non-emergency google form and request a counselor check-in. 

Parent Referral (English) (Español): Parents can submit a referral form to initiate a counselor check in with their student. 

Staff Referral: Staff can complete a referral for a student of concern to the counseling department.
